Living Well With Diabetes - Education for Self-Management

This course consists of four sessions in a group setting. Day and evening classes are available. Your support person is encouraged to attend. Classes are taught by a Registered Nurse (R.N.) and a Registered Dietician.

Cost/ Coverage

• $580/person. Physician referral is required. Usually covered by medical insurance or medical coupons.
• Usually covered partially or fully by Medicare, Medicaid, and other insurances with a physician referral. Check with your insurance.
